Skilled Nursing and Rehabilitation
Interdisciplinary teams provide a full range of support and treatment to residents. Each of our Skilled Nursing and Rehabiliation facilities offer Rehabilitative Therapy, Nutritional Services, Social Services and many other necessities. Assisted Living
Assisted Living is a senior housing solution for adults who can live independently, but need some assistance. We are affiliated with LC Geri Care, located in New Castle, PA. To learn more, view their site.
Personal Care
Personal care homes are designed to provide safe, comfortable and supportive residential settings for adults who do not require the services in or of a licensed long-term care facility, but who do require assistance or supervision with activities of daily living. Our Lakeview location offers a homelike environment focused on assisting our residents to develop and maintain maximum independence.

Dementia Care
Dementia Care, including Alzheimer's type is available at the Memory Support Center. The center was designed to provide a flexible and secure living environment for residents with memory disorders. Special steps were taken in building the center to compensate for its' resident's cognitive losses.

Independent Living
Independent Living is the perfect option for active seniors who would prefer to live without the burdens of household maintenance. Generous amenities are provided at both of our Independent Living locations.

Home and Transport Services
With home care and transport services, our mission is to provide quality care while ensuring a safe and comfortable environment for those we serve. Quality caregivers are available to serve your needs.

Outpatient Therapy
We offer Outpatient Therapy to treat individuals with musculoskeletal disorders and work-related injuries. Outpatient Therapy is also ideal for post-surgical, orthopedic and stroke patients. Our experienced, licensed therapists work with patients and use resistance training and a variety of exercises to aid in recovery.
